Guidelines
to apply for therapy funding through SIERF
Parent will contact
therapist who, following a records review and initial intake will:
1. Evaluate and recommend an
intervention plan including in-clinic and home based goals.
2. Will develop measurable goals and time lines, demonstrating
effectiveness of therapy.
3. Will estimate the cost of therapy.
4. Will submit notes summarizing treatment and goal progress on
a monthly basis.
Parent will:
1. Submit income W2s or latest tax return.
2. Submit a plan of continuance after funding. |
